Documents required for admission
In order to quickly process your admission and get you settled
in without delays, the following documents are vital for your
admission.

Proof of identity of the patient
• Proof of identity of the principal member of the
medical
aid scheme if he/she is not the patient
• Your medical aid membership card
• Pre-authorisation number from your medical aid
• All X-rays, scans, blood tests or other test
results which
preceded admission.
• A signed consent form from your admitting doctor

Preparation for Surgery
• Do not eat or drink anything (including water) for at least
six hours prior to your scheduled surgery. If you
are required to be nil per mouth prior to your procedure, please
discuss taking your regular medication
with your doctor. When taking your regular medication, try to
swallow dry or with as little water as
possible.
• Please bring all current medication with you to hospital,
preferably in its original packaging.
• Do not wear jewellery, make-up or nail varnish on the day of
your surgery.
• For day patients, please ensure that you have someone to take
you home after your procedure. Driving a
vehicle following anaesthesia or sedation is not recommended.
